29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm) Location: 198 Tecumseh St. Fall River MA On or about March 4th 2005, employees were exposed to falls to the ground from heights up to 24 feet to the ground while working on a pump jack scaffold system where the planks did not extend pass the centerline of the supports.
