29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) On or about February 21, 2025, one or more methods of machine guarding was not provided to protect the operator of a TCM Solutions Winder 50-450T (Line 8) from hazards created by the rotating belts of the machine when the operator cleaned a belt while the machine was in operation. This violation resulted in an injury when the operators right arm entangled in the belt and the paper wrapped mandrel. B) On or about February 26, 2025, one or more methods of machine guarding was not provided to protect the operator of a TCM Solutions Winder 50-450T (Line 8) from hazards created by the rotating belts of the machine when the operator placed their hand on the paper roll product to manually feel for accurate wrap spacing within approximately 10" of the hazard area while the machine was in operation.
