29 CFR 1910.134(i)(1)(ii)(A): Breathing air did not contain 19.5-23.5% oxygen (volume/volume): On or about February 24, 2025, in Plant III, employee(s) were exposed to asphyxiation and death in that the compressed air system did not meet requirements for Grade D breathing air in the following instance: 1) In Paint Booth 5 the supplied air hood had oxygen levels recorded at 5.3% concentration. 2) In Paint Booth 7 the supplied air hood had oxygen levels recorded at 7.8% concentration 3) In the Carousel/Prime Booth, the supplied air hood had oxygen levels recorded at 5.3% concentration.
