29 CFR 1910.28(b)(3)(v)(A): The employer did not ensure each employee was protected from falling through a hatchway as required and/or when the hatchway was not in use, the employer did not ensure the cover was closed or a removable guardrail system was provided on the exposed sides. (a) On February 15, 2025 and times prior at 170 GTA Drive, Dickinson, ND, an employee was exposed to a fall hazard of greater than four feet when working on a second level of a tower freezer performing maintenance when a hatch on the floor was left open and no guardrail were in place.
