29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): Site: 88-40 54th Avenue, Elmhurst, NY 11373. a). On or about February 19, 2025, Tri State Plumbing & Heating Inc. employees engaged in the excavation of a trench for a plumbing line at a depth of approximately 8 ft deep, 4 ft wide and 7 ft long were not protected from cave-in hazards by the use of shoring or other protective systems.
