Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(3)(i): When a tagout device is used on an energy isolating device which is capable of being locked out, the tagout device shall be attached at the same location that the lockout device would have been attached, and the employer shall demonstrate that the tagout program will provide a level of safety equivalent to that obtained by using a lockout program. a. On or about February 13, 2025, the employer failed to demonstrate that the tagout program outlined in the Plant Clearance Permit Procedure provided a level of safety equivalent to that obtained by using a lockout device. This condition exposed employees to caught-in hazards from the rotating parts and pinch points during servicing activities to include but not limited to the of the Emergency Gypsum Conveyor 1 (EGC1) line. Additional safety measures were not used in conjunction with tagging an energy isolation device to prevent the unexpected re-energization such as but not limited to, the removal of additional isolating circuit element-fuse, to provide a second layer of protection in the event the tagout device for the primary isolation was defeated.
