29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. On or about 02/13/2025, at 6450 NW 25th Street, Bldg. 704, Miami, Florida, employees from a crew performing pressure washing duties and another crew removing an existing roof and placing debris in garbage bags were exposed to 25-foot fall hazards when working in close proximity to skylights.
