29 CFR 1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: At a worksite located at 1031 North Topping Avenue Kansas City, Missouri, this was most recently documented on or about February 11, 2025, and at times prior the employer exposed their employees to respiratory, atmospheric, and grain engulfment hazards. The employer did not ensure that signage or other effective means were used to warn employees of the danger posed by the accessible permit required confined spaces throughout the bin deck floor, distributor floor, scale floor, and etc. located throughout the elevator. NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
