Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to struck-by hazard. On or about February 10, 2025, and at times prior to, in the warehouse, employees were exposed to struck-by hazards from falling storage rack systems and debris while operating a forklift to place pallets and products up to three tiers high in four rows of a pallet rack system. The four southernmost pallet racks had 14 supports each and none were secured with anchors. The next two racks had 20 supports and none were secured with anchors. The northmost pallet rack had 20 supports with 16 secured with anchors. Pallet racks without proper anchoring and securement to the floor are known to collapse due to contact or misaligned loads.
