1926.451(f)(3) Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Job site, intersection of I 70 and Route 51, Belle Vernon, PA: On or about January 30, 2025, during construction activities, the employer did not ensure appropriate guardrails systems were in use on a scaffold at the abutment pier, exposing employees to fall hazards of up to approximately 15 feet.
