29 CFR 1910.28(b)(3)(ii): Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems; On or about January 29, 2025, at the establishment located at 2441 East Glendale Avenue, Appleton, Wisconsin; the employer did not ensure employees working near the rotary wrapping arm inside the spider wrapper (Roll Wrap System P.O. 111767) were protected from tripping or stepping into a 32-inch deep floor hole where the ejection rollers were installed into the concrete floor. Employees securing the plastic wrap to paper rolls were exposed to slips and falls onto the same level as the hazard.
