At the worksite, located at 4415 75th St W Bradenton, FL 34210; On or about 1/28/25, the employer exposed employees to entrenchment and asphyxiation hazards in that cave in protection was not provided for employees conducting excavation operations approximately 12 ft in depth while working on underground utility sewer lines.
