Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 21, 2025, the employer did not ensure the developed and documented hazardous energy control (lockout/tagout) procedure was utilized for the control of the hazardous electric and pneumatic energy sources when machine operator employees were engaged in service and maintenance activities, such as unjamming the 804 Line Palletizer Machine. Machine operators reach into and/or enter into the machine to unjam product cases (boxes) without utilizing the developed and documented hazardous energy control procedure, exposing them to the unexpected startup of the palletizer machine.
