In the manufacturing area, ACCUR Shear machine Model No.625012, Serial No.7789: On or about January 28, 2025, employees were exposed to caught-in-hazards, in that, the employer did not establish energy control procedures specific to each type of machine which may be Locked or Tagged out. b). In the manufacturing plant, Piranha Ironworks Machine, Serial No.9381: On or about January 28, 2025, employees were exposed to caught-in-hazards, in that, the employer did not establish energy control procedures specific to each type of machine which may be Locked or Tagged out. c).
