29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. (a) 1955 W. North Ave., Melrose Park, IL 60161 - On or about January 22, 2025, the employer did not ensure that employees were able to use a safe means of access and egress to and from walking-working surfaces within the driveway of the automated racking structure operated by Symbotic, LLC to access levels 6 and 7 accessible by each driveway. Employee(s) used structural components on opposing sides of the open area of the driveway as steps to access the levels to complete a driveway reset. (b) 1955 W. North Ave., Melrose Park, IL 60161 - On or about January 24, 2025, the employer did not ensure that employees were able to use a safe means of access and egress to and from walking working surfaces including the transfer deck of level 11 of the Symbotic, LLC operated automated racking structure, accessible from a platform installed in the transfer deck access area within zone 3. Employees used structural components of the railing system to access the transfer deck, located approximately 20 inches above the railing around the platform. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
