29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. On or about January 22, 2025, the employer failed to ensure that the LP Toyota Forklift Model: 8FGU32, Serial Number: 12836, and LP Toyota Forklift Model: 42-5FG25, Serial Number: 405PG25-15941, with front-end fork attachments other than the factory installed attachments identified the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, thereby, exposing affected employees to tip over hazards.
