At 2654 Stonegate Dr. Jacksonville, FL: On or about January 20, 2025, the employer did not provide fall protection training that included the use and operation of personal fall arrest systems to roofing employees exposed to fall hazards up to 10 feet to the ground below while re-roofing a residential house.
