Text For Citation: 01 Item/Group: 001 Hazard:

OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to anhydrous ammonia contact, asphyxiation and explosion hazards from potential equipment failure related release from the failure of ammonia refrigeration components due to ice build-up and icing: (a) On or about January 13, 2025, Engine room LPR-1 vessel: Access to isolation/shutoff valves were not readily accessible in-that the isolation/shutoff valve hand wheels were encapsulated with excessive ice/frost build-up and did not allow immediate access to the vessels isolation/shutoff valves. (b) On or about January 13, 2025, Engine room INT-1 vessel: Insulation on left side of the INT-1 piping (IC-21) had the insulation removed from the tank to the main shut-off valve, creating excessive ice/frost build-up on the pipe and the sight glass.
