29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a) On or about November 23, 2024, at 9325 SW 108th Street, Miami, FL 33176, an employee leveling the surface of poured concrete inside a beam form was exposed to a fall hazard of 13 feet, in that; no means of fall protection or restraint was being used.
