29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A) 701 Montrose Ave South Plainfield, NJ 07080 The employer failed to ensure each operator had been trained evaluated on the forklift. The employer failed to provide documentation which included the name of the operator, the date of the evaluation, and the identity of the person performing the evaluation. This condition exposed employees to struck by hazards. Hazard noted on or about 12/30/2024
