29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years. a. On or about December 30, 2024, in the facility, employees that received initial forklift operator training were not evaluated every three years to access the employee's performance of safely operating powered industrial vehicles. Employees were thereby exposed to struck-by and crush-by hazards. Jewel Food Stores, Inc. a division of New Albertson's, L.P. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1636623, citation number 1, item number 1 and was affirmed as a final order on February 8, 2023, with respect to a workplace located at 1955 W. North Avenue, Melrose Park, IL 60160.
