29 CFR 1926.21(b)(2): Employees did not follow their training for the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury. On or about December 19, 2024, and at times prior thereto, employees were exposed to struck-by hazards while loading heavy equipment parts onto a trailer that was not sitting level and stacking the parts in an unstable manner. The employer did not ensure training to recognize, avoid, and control unsafe conditions.
