29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): (a) On or before December 17, 2024 and at times prior, the employer did not ensure that the open holes on the tilt deck of the furnace, in an area referred to as "no man's land," were covered or that employees accessing the area were protected from fall hazards. An employee accessing this area to place insulation wool on a burner fell through one of the three unguarded holes 21 feet to the concrete floor beneath and suffered serious injuries as a result. CF&I Steel, LP dba Evraz Inc. NA was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.28(b)(3)(i), which was contained in OSHA inspection number 1719340, citation number 1, item number 1, and was affirmed as a final order on 6/6/2024, with respect to a workplace located at 2100 SOUTH FREEWAY, PUEBLO, CO 81004.
