Willow Avenue and Sewell Avenue, in Colby, Kansas, on December 3, 2024, employee(s) engaged in trenching and excavation activities were exposed to cave in and engulfment hazards in that the employer did not provide training or instructions to employee(s) regarding the recognition and avoidance of unsafe conditions such as cave ins and engulfment.
