OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by hazards: a.) On or about December 5, 2024, and at times prior thereto, located in the abrasive blasting booth, employees were exposed to a crushed-by hazard while performing abrasive blasting on metal sheets that were positioned in free-standing, shop-made fixtures with slots that allowed the metal sheets to lean. b.) On or about December 6, 2024, and at times prior thereto, located in the main shop, employees were exposed to a crushed-by hazard while utilizing an overhead crane to move loads and the hoists open-throat hook was missing the safety latch.
