29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about December 3, 2024, at 3338 Congen Road, Labelle, Florida, the employer did not instruct employees on the proper lock-out/tag-out (LOTO) procedures to be followed when cleaning a Rapid-Mix 400 CW mixer machine, including the training to recognize and avoid the hazards associated with cleaning of such mixer.
