OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards associated with relief valve venting: On or about November 20, 2024, a worker near the 2-phase separator ves- sel at the Akers 968 C001BO during hot oil operations for paraffin buildup failed to wear a hard hat despite being trained to don a hard hat. This exposed the worker to struck-by hazard from the 10-foot-long pressure release/pop-off valve vent pipe that terminated to an open 90-degree elbow fitting.
