29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident On November 18th, 2024, an employee collapsed and was immediately transported to the hospital where they died shortly after arriving. The fatality was not reported to OSHA until it was referred to the Honolulu Area Office by the Guam Office of the Chief Medical Examiner on November 21, 2024.
