Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(6)(i): The control of hazardous energy (lockout/tagout). General. The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (A) The periodic inspection shall be performed by an authorized employee other than the ones(s) utilizing the energy control procedure being inspected. (B) The periodic inspection shall be conducted to correct any deviations or inadequacies identified. (C) Where lockout is used for energy control, the periodic inspection shall include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. a) On or about November 8, 2024, the employer did not conduct periodic inspections of each energy control procedure at least annually with the Maintenance Technicians for equipment including but not limited to the 5A2 elevator lift conveyor at the NP2 Sub Assy Steel Ball Force Fit Machine to ensure that procedures were adequate to prevent exposure to moving parts on the NP2 Sub Assy Steel Ball Force Fit Machine when changing belts and when checking the chain and sprocket for tension. There was no review of the authorized employees' responsibilities to lockout energy sources such as the electrical disconnect, gravity, and pneumatics to prevent caught in hazards when performing maintenance on the NP2 Sub Assy Steel Ball Force Fit Machine. b) On or about January 10, 2025, the employer did not conduct periodic inspections of each energy control procedure at least annually with the Maintenance Technicians for equipment including but not limited to the Horkos Machining Center on the Subaru Line 3 OP-10 #1 to ensure that procedures were adequate to prevent exposure to moving parts as well as pressurized coolant and air located at the Horkos, when cleaning the sensors at the panel inside the spindle/machining area and cleaning filters of metal inside the mist eater. There was no review of the authorized employees' responsibilities to lockout energy sources such as the electrical disconnect, gravity, pressurized coolant lines, and pneumatics to prevent exposures to air, coolant, and caught-in hazards when performing maintenance on the Horkos Machining Center.
