OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a) At 11325 County Road 44, Leesburg, FL 34788, Concrete Crusher Area; On or about 11-18-2024, the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a fall hazard of approximately 6 feet 10 inches to the ground below while ascending and descending a metal single (straight) ladder for the picker station platform. The 75.5 degree angle of inclination for safe positioning of the ladder was not maintained to prevent accidental displacement. b) At 11325 County Road 44, Leesburg, FL 34788, Concrete Crusher Area; On or about 11-18-2024, the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to a fall hazard of approximately 6 feet to the ground below while ascending and descending a metal single (straight) ladder for the impact vibrator, crusher machine platform. The 75.5 degree angle of inclination for safe positioning of the ladder was not maintained to prevent accidental displacement.
