29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) The Hershey Company Reese Plant 925 Reese Avenue Hershey, PA - On or about November 15, 2024, the Clark Side Load Forklift #22, model ECX360, serial number ECX360-2745-9653KF, had defects, including oil leaks and a missing bolt head on the carriage plate. This powered industrial truck was not withdrawn from service for repairs. b) The Hershey Company Reese Plant 925 Reese Avenue Hershey, PA - On or about November 15, 2024, the Clark Side Load Forklift #23, model ECX360, serial number ECX360-2746-9653KF, had defects, leaking hoses, and a damaged hose track, and this powered industrial truck did not get withdrawn from service for repairs. c) The Hershey Company Reese Plant 925 Reese Avenue Hershey, PA - On or about November 15, 2024, the Yale Reach Truck #24, model NR035DB, serial number B295N04743S, had defects, including oil leaks and an active coil circuit error code. The forward roller wheel on the stabilizer leg was mounted with the incorrect hardware, and this powered industrial truck was not withdrawn from service for repairs. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
