29 CFR 1910.147(c)(8):Lockout or tagout was not performed by the authorized employees who are performing the servicing or maintenance: a) On or 11/13/2024 in the production area; where employees perform servicing and/or maintenance activities such as, but not limited to: changing dies on the PIRANHA iron worker without applying his/her own lock on the energy isolating means. An employee, different than the employee performing the servicing/maintenance, put a lock on the disconnect switch. ABATEMENT CERTIFICATION REQUIRED
