Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard from a 2000 ton to 5000 pound mold falling during the hoisting operations due to a chain failure: (a) The mold setter was using 3/8 inch carbon steel chain slings to hoist 2000 to 5000 pound molds out of the 29 injection molding machine, on or about April 17, 1991. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use alloy steel chain for all slings used for hoisting equipment. All alloy steel chain slings shall have tags that identify the size, grade, rated capacity and reach for each chain sling. A monthly inspection shall be conducted for each alloy steel chain sling.
