29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): At a worksite at 626 E. Steel St., Seymour, MO 65746: Workers were exposed to struck-by and crushing hazards. This occurred when employees were permitted to operate powered industrial trucks that were found to need repair, had defective components, and was unsafe to other workers. The equipment was not taken out of service and remained in an unsafe operating condition. On or about November 17, 2024, a Clark - powered industrial truck (Model#EC120/Serial#E9120019-5391FA) was found to be in need of repairs and was permitted to stay in operation near the laser department of the facility. The operational controls of the equipment had been damaged and/or were missing or illegible. Hutchens Industries Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1577782, citation number1, item number 4 and was affirmed as a final order on ( August 16, 2022 ), with respect to a workplace located at 898 E. Commercial, Mansfield, MO 65704.
