29 CFR 1926.21(b)(2): The employer did not instruct each employee to recognize and avoid unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or around November 5, 2024, a Compliance Safety and Health Officer (CSHO) observed employees who were not trained in safe operations, safe conditions, and recognition of unsafe conditions while working from a scaffold.
