29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces: On or about November 11, 2024, the employer failed to ensure that employees had a safe means of access at the box erector machine when they had to step over the conveyor to get from side to side. The conveyor was laying on a metal grate single step which did have rails, but the conveyor was on top of the grate and presented a tripping hazard.
