29 CFR 1960.8(a):The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm. a) On or about November 2, 2024, employees were exposed to fall and struck-by hazards while riding as the operator and passengers. Safe operating practices were not followed, including the use of seat belts provided by the Polaris Ranger XP 1000 manufacturer. An employee suffered serious head injuries after falling from the vehicle without wearing a seatbelt, resulting in the employee's death. b) On or about November 1, 2024, employees were exposed to fall and struck-by hazards while riding in the cargo (bed) box of a Polaris Ranger XP 1000.
