CFR1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft,(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT): a. On or about November 2, 2024, the employer did not ensure that employees performing cutting on a section of pipe and welding on a flange were protected from the unexpected release of hot condensate from a steam pipe exposing the employees to thermal burn hazards.
