29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Envases Ohio, LLC. - Fremont, Ohio: On or about October 31, 2024, the employer did not ensure the rollers and pneumatic powered conveyor on line #1 were deenergized prior to the performance of service and cleaning work. Employees tasked with cleaning these rollers in an energized state, were exposed to crushing and caught-in hazards at the rollers and conveyor section near output side of roll coater.
