29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about October 31, 2024, a stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for an employee that entered a trench that was approximately 77.5 feet long by 3 feet wide with the deepest measured spot in the trench being 6.5 feet to install a tracer wire for locating drainage pipes which were being installed for a golf course development at Grand Geneva Resort and Spa in Lake Geneva, WI.
