At the warehouse, located at 6 Gum Street, in Fernandina Beach, FL, 32034: On or about October 7, 2024, the employer exposed employees to struck by and crushed by hazards, in that the employer did not ensure that parking brakes and rear wheel chocks of highway truck were set to prevent truck from rolling while performing loading operations.
