Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Basement First and Second Elevator Floor Levels Employee(s) utilizing the plant "Freight" elevator, to transport company products, from one to any of three industrial plant floor levels were exposed to serious injuries. The three landing(s) elevator hoistway doors were not provided with an interlock system, which would prevent any employee from opening the hoistway door(s) unless the elevator is stationed at that landing. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Equip each set of hoistway doors on all three levels with hoistway unit system hoistway door interlocks, preventing any door from opening, unless the elevator is at the level called from. 2) The hoistway unit system shall interlock the doors in a closed position before the elevator ascends or descends to another level.
