29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) MAC Custom Processing LLC dba Hasselbach Meats on or about October 19, 2024, at the address of 4637 Oak Harbor Rd. Fremont, OH: The employer failed to maintain an energy control program that provided for instruction and procedures for energy isolation in the performance of equipment service/cleaning tasks. Employees tasked with cleaning equipment such as but not limited to the Hollymatic model 120 Precision Bulker without the benefit of an effective energy control program and isolation techniques were exposed to amputation hazards.
