Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees were exposed to a crushing hazard from the hydraulically controlled gate in the Bambury machine during cleaning operations. One feasible method of abatement, among others would be to electrically interlock the hydraulic pump and the drain valve withthe mixer gate control so that all systems on machinery are at a zero energy state while cleaning or maintenance is being performed.
