29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: (a) Excavation site located on the shoulder of WI-37, Mondovi; On or about October 16, 2024, only three of the four required spreader bars were installed between the sidewall panels of a 24-foot long x 8-foot high trench shield before it was installed and used to provide cave-in protection in an excavation that ranged in depth from 10-feet, 11-inches to 10-feet, 5-inches.
