29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about October 11, 2024, and dates prior, at the (A3) Tian Wei perforating machine, employees work at and around the point of operation of the machine while feeding coil aluminum into perforating machine without adequate point of operation guarding in place. This exposed employees to a caught-in, crushing, and amputation hazard.
