29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) Workroom floor: On or about 10/7/2024 and at times prior to, the employer exposed employees to struck by hazards, in that forklifts training was not provided to employees operating forklifts while moving and other PIT related hazards, in that forklift operators were observed driving PITs in the same area where employees were walking and working, and had not been evaluated as required.
