29 CFR 1926.20(b)(2): Such programs had not provided for frequent and regular inspections of the job sites, materials, and equipment to have been made by competent persons designated by the employers. On or about October 2, 2024, Zykan Exteriors STL LLC did not ensure that adequate frequent and regular inspections of the job site by competent persons had occurred at 1131 O'Fallon Lakes Drive in O'Fallon, Missouri, thus exposing employees to the following hazards: A) At least three employees were exposed to fall hazards from roofing a three-story apartment building without utilizing fall protection. B) At least three employees were exposed to fall hazards from utilizing a portable extension ladder that was not properly positioned. Employees were not provided a grasping device, such as a grabrail, to assist them in mounting and dismounting the ladder. C) At least four employees were exposed to struck-by and falling object hazards from working on the lower level, below roofing activities, without head protection.
