29 CFR 1926.100(a): Employees working in areas where there had been a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, had not been protected by protective helmets. On or about October 2, 2024, California Exteriors LLC did not ensure that four employees working from the ground level below roofing activities were protected from falling or flying objects by utilizing protective helmets in a worksite at 1131 O'Fallon Lakes Drive in O'Fallon, Missouri.
