29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a) Adler Pelzer Group on or about October 15, 2024, at the location of 1550 N. Wynn Rd. Oregon, Ohio 43616: The employer did not ensure open floor holes were effectively guarded or covered on the Manual Line. Due to this condition, employees operating the Manual Bruno press in proximity of these holes were exposed to a tripping/falling hazards.
